What the forms collect

The app interest form asks for an email address. It also records that you requested app-related updates, a signup source label, and a submission timestamp. The Med Station Box form records an email address, box reservation interest, source, and timestamp. Neither form asks you to enter treatment records or health details.

Why it is collected

FineWino LLC uses this information to manage app interest and box reservations, and to send relevant availability or order updates. A form submission saves your interest; it does not take payment, guarantee a product or beta place, or automatically send a confirmation email.

Storage and hosting

Submissions are stored in the website’s hosted database. Website hosting and infrastructure providers process data needed to serve this site and receive your form submission. This website does not provide access to the app’s private treatment records.

Your choices

You can choose not to submit a form. To request removal from an interest or reservation list, ask about your submission, or request deletion, contact support@finewino.com. Please do not include medical records in your message. A specific automatic deletion period has not been established.

The site includes fictional app-design previews, links to FineWino, and a downloadable resource. External sites have their own privacy practices. Rooted in Hope is for organization and emotional support and does not replace your clinic’s treatment instructions.
